AFFIDAVIT OF ELIGIBILITY
AND RELEASE OF LIABILITY
I represent and certify to PRODEGE, LLC ("COMPANY") that I am a legal resident of the United States, The United Kingdom, or Canada and that I am 13 years or older. I hereby certify that I am eligible, under the contest rules attached as Attachment A hereto (the "Rules"), to claim the applicable prize set forth below and further described in the Rules (the "Prize"). I represent that I have read, fully understand and agree to be bound by the Rules.

COMPANY may use my name, likeness and/or voice in connection with advertising and/or promotional activities related to this contest on COMPANY's Web site, as well as in other promotional material that may be developed and distributed in any medium by COMPANY. COMPANY & it's branded partner may send me emails with updates regarding this promotion.

I agree to release and hold COMPANY and COMPANY's employees, officers, directors, shareholders, and agents harmless from any and all losses, damages, rights, claims and actions of any kind resulting from or relating to the acceptance, possession, or use of the Prize, including without limitation, personal injury, death, and property damage, claims based on publicity rights, defamation, invasion of privacy, or any other claim which I now have or may hereafter have.
